    About Us

    As Wellmont Health System's new accountable care organization - or ACO - the Wellmont Integrated Network is a health services organization made up of the primary care physicians and specialists of Wellmont Medical Associates and the Wellmont CVA Heart Institute, along with Wellmont's hospitals and other facilities. The network serves Medicare Fee-for-Service patients in Kingsport, Bristol, Johnson City and surrounding communities in Northeast Tennessee and Southwest Virginia.

    These individuals and organizations work together to provide you quality health care with an emphasis on striving to keep you healthy instead of solely providing care when you are sick or injured.

    The Wellmont Integrated Network is working with Fee for Service Medicare as part of the Medicare Shared Savings Program.

    What is the Wellmont Integrated Network?

    As Wellmont Health System's new accountable care organization (or ACO), the Wellmont Integrated Network is a health services organization made up of the primary care physicians and specialists of Wellmont Medical Associates and the Wellmont CVA Heart Institute, along with Wellmont's hospitals and other facilities.

    These individuals and organizations work together to provide you quality health care with an emphasis on striving to keep you healthy instead of solely providing care when you are sick or injured. The Wellmont Integrated Network serves Medicare Fee-for-Service patients in Kingsport, Bristol, Johnson City and surrounding communities in Northeast Tennessee and Southwest Virginia.

    How will you benefit from the Wellmont Integrated Network?

    To enhance the care we provide, the Wellmont Integrated Network is committed to improving the coordination of care among our physician practices, hospitals, outpatient centers and other facilities. If you are currently under the care of one or more of our physicians, you may be contacted by a case manager to assist in coordinating your care.

    We believe your relationship with your doctor is very important, as is your doctor's relationship with other physicians and caregivers who may be helping provide your care. Enhancing communication between doctors, nurses and other staff across our facilities, along with maintaining clear lines of communication with you when you are a patient, will help ensure you receive quality medical care.

    I received a letter saying my doctor is in the Wellmont Integrated Network. What do I need to do?

    You don't need to do anything. Even though your doctor is participating in the Wellmont Integrated Network, your Medicare benefits won't change. You'll still be in Original Medicare, and you'll still have the right to use any doctor or hospital that accepts Medicare, at any time.

    May I continue to see my doctor? 

    Yes. We assure you that your relationship with your doctor will not change. You may continue to see your doctor and other caregivers as you have been doing with no interruption in service.

    What about other doctors, including those who aren't part of the Wellmont Integrated Network? May I see them? 

    The Wellmont Integrated Network does not restrict who you can see or to whom your doctor may refer you. You may still see any healthcare provider who accepts Medicare, including those outside the Wellmont Integrated Network, at any time.

    Is the Wellmont Integrated Network just another HMO?

    The Wellmont Integrated Network is an accountable care organization (ACO), not an HMO. You may go to any hospital and continue seeing any provider that accepts Medicare, whether or not they are part of the Wellmont Integrated Network.

    Will this change my Medicare benefits?

    No, this will not affect your Medicare benefits at all. You will still be in Original Medicare and continue to receive your benefits through Medicare as before.

    Does my doctor's participation in this network change how he or she is paid?

    No, your doctor's compensation is not affected by his or her participation in the Wellmont Integrated Network. And the services you receive will not change either. The way Medicare reimburses your doctor for services is the same as before, and your process for submitting Medicare claims will also remain the same.

    The Wellmont Integrated Network does have the opportunity to share savings with Medicare if the organization meets several quality benchmarks and costs of services provided are reduced. But that money would be reinvested in advancing health care throughout Wellmont and its physician practices.

    Does this have anything to do with my healthcare information?

    We are making significant improvements to how information is shared between our caregivers, and we are confident these improvements will have a positive impact on the health care you receive. You can rest assured, however, that the only people who have access to your healthcare information are your doctor and any other caregivers who are specifically providing you with care - no one else. Your medical information will remain private and secure. 

    What about my privacy?

    Your privacy is very important to us. We want you to know that Medicare shares certain health information with the Wellmont Integrated Network. This information will include things like your visits to the doctor or hospital, your medical conditions and any prescriptions you've had in the past or are taking now. The reason Medicare shares this information is because having the most up-to-date information about your health will help improve the care we provide to you.

    Like Medicare, we have safeguards in place to make sure all your medical information is protected.

    Regardless, you control the use of your personal health information, and you do have the option to decline having certain personal information shared between Medicare and your doctor, Wellmont Medical Associates, the Wellmont CVA Heart Institute or the Wellmont Integrated Network. Please fill out and submit the "Declining to Share Personal Health Information" form if you prefer. You should have received a copy of this form in the mail already.

    How do I decline having my health information shared?

    There are two ways to decline having Medicare share your health information with your doctor or the Wellmont Integrated Network.

    1. Complete and submit the "Declining to Share Personal Health Information" form to the Wellmont Integrated Network at the following address:

        You should have received a copy of this form in the mail already, or you may pick up a copy at your doctor's office.
       
    2. Call Medicare at 1-800-MEDICARE (1-800-633-4227) and say that you wish Medicare to stop sharing your personal information with the Wellmont Integrated Network. TTY users should call 1-877-486-2048.

    Please see the next question for more about what Medicare may and may not communicate to your doctor or the Wellmont Integrated Network if you decline to have your information shared.

    What happens if I decline having my health information shared?

    If you submit the "Declining to Share Personal Health Information" form, Medicare will stop sharing with the Wellmont Integrated Network information about your visits to the doctor or hospital, your medical conditions and the prescriptions you've had in the past or are taking now.

    However, you should be aware that even if you do not want to share your personal health information with your doctor or the Wellmont Integrated Network, Medicare will still use your information for some purposes, including certain financial calculations and in determining the quality of care provided by your doctor and the Wellmont Integrated Network. Also, as part of assessing the quality of care we provide, Medicare may share some of that information with us.
