• Wellmont Medical Associates Is an Acclaim Award Honoree

    Wellmont Medical Associates has been named to an exclusive list of medical groups honored with the American Medical Group Association's 2014 Acclaim Award.

    AMGA recognizes a select few groups each year in its Acclaim Award program, and Wellmont Medical Associates is the only honoree in our region. In fact, Wellmont Medical Associates is just the second group ever to be recognized with an Acclaim Award across Tennessee, Virginia, North Carolina, Kentucky and West Virginia.

    This achievement puts Wellmont Medical Associates in the company of widely recognized and respected organizations such as Mayo Health System, Cleveland Clinic Foundation, Henry Ford Health System and Geisinger Health System.

    Why were we honored with an Acclaim Award?

    AMGA recognized Wellmont Medical Associates for keeping patients healthier and out of the hospital. A few of our more significant accomplishments to enhance patient care and outcomes include:

    • Achieving an overall hospital readmission rate of less than 11 percent, much lower than the national average of 17 percent
    • Demonstrating a readmission rate of just over 5 percent for one of our Medicare Advantage contracts, an achievement we intend to match across our Medicare patient population
    • Reaching the highest percentile for safe prescribing by successfully eliminating or avoiding high-risk medications when appropriate
    • Providing access – same-day in many cases – to primary care services for 95 percent of our patient population, exceeding the Medicare Advantage target of 85 percent
    • Ranking in the 80th percentile for performing comprehensive health screenings on managed care patients older than age 65, far exceeding the 55th percentile goal

    How did we do it – and what does this award mean for you?

    There are a number of benefits patients will experience when they visit Wellmont Medical Associates. We have fully embraced a physician leadership model, which has driven our patient-centered approach to care and many other important changes.

    Wellmont Medical Associates has made significant progress in managing diabetes, controlling cholesterol in our diabetic patients and controlling hypertension – just a few examples of the improved results for our patients.

    Another key factor is the new MyWellmont patient portal, which offers Wellmont Medical Associates patients easy, secure access to their health records, eliminating duplication of services and enhancing the quality, safety and efficiency of our care.

    And in making these improvements, we're seeing our highest levels of patient satisfaction ever. By the end of 2013, we ranked in the 78th percentile of all groups nationwide - and that number continues to climb. Some of our practices have achieved the 99th percentile nationally.
